Risk adjustment calculations involve complex rules, encounter data, and patient demographics that are beyond the scope of this tool. All risk adjustment coding should be reviewed by a qualified RAF coder or risk adjustment specialist.

5. ICD-9 to ICD-10 Crosswalk

The ICD-9 to ICD-10 crosswalk mappings are based on CMS General Equivalence Mappings (GEMs) and are provided for general reference. GEM mappings are approximate and may not reflect the most clinically appropriate code for a given situation. Many ICD-9 codes have multiple valid ICD-10 equivalents depending on clinical documentation. Always consult official coding guidelines and clinical documentation when converting codes.

The ICD-10-CM dataset used on this site is sourced from CMS Section 111 Valid ICD-10 FY2026, effective October 1, 2025. This data is published by the U.S. government and is in the public domain.
